Primary care system-level support

• Providing PCNs with a tool that allows them to easily access their operational data in real-time (Apex) and help them to understand and validate this data.
  
• Taking PCNs through improvement cycles with a real project to unlock real benefits.
• Building capability, understanding and data standards at PCN level to enable practices to begin to harness the benefits of working together.

Following the addition of the role of a Digital and Transformation Lead to the list of roles reimbursable to PCNs through the ARR scheme, we are providing support to PCNs and their practices to design and implement ongoing improvement to services, including the co-design, development and delivery of capacity and access improvement plans as part of the 23/24 Capacity and Access Programme.

Commissioning of this service at a place- or system-level will deliver efficiencies, enable access to a range for relevant skills through our blended approach and enable a system view of priorities and plans.  

This aggregated view is critical to informing system strategies to ensure gaps in health inequality and inequity are not widened through stand alone initiatives.

• Booking, changing or cancelling an appointment (including using online consultation forms and NHS App interfaces)
• Ordering repeat prescriptions
• Registering as a new patient; including finding out key info such as phone number, address and opening hours and explaining access arrangements
• Getting test results
• Getting a fit note for work

• A remote digital assessment using national and digital maturity tools to explore a practice’s digital media
• Completion by the practice digital lead of a short self assessment of their digital offering against ‘what good looks’ like criteria. This is a tool used to identify the gap between reality and perception and to indicate how much knowledge enhancement to factor into the PCN improvement plan.

Understanding areas for neighbourhood level working to support the management of demand on general practice through the completion of an electronic audit of avoidable appointments.

Not only would this serve as an indication of priority services for neighbourhood approaches but also as a baseline metric of impact of future developments.

Our IT training team will download the software to work alongside the clinical system ensuring that the set up is optimised for the practice. A ‘How to’ video will be available to support users.
