Our approach builds on our experience in primary care development with the ability to draw in expertise as required from specialists in areas such as care analytics, organisational and leadership development, communications and engagement, evaluation and change management. 

  • Integrated Neighbourhood team development - Facilitating the rapid adoption of population health management approaches to allow PCNs to re-imagine care pathways and develop Integrated Neighbourhood Teams.
  • Implementation and evaluating impact  - Support Level Framework implementation and review - Measure and evaluate the impact of actions that primary care, PCNs and systems have taken to date.

Support available

Through our ELITE programme:
  • Providing PCNs with a tool that allows them to easily access their operational data in real-time (Apex) and help them to understand and validate this data.
  • Taking PCNs through improvement cycles with a real project to unlock real benefits.
  • Building capability, understanding and data standards at PCN level to enable practices to begin to harness the benefits of working together.

At-scale support

  • Review and alignment of locally commissioned services.
    As CCGs merged to form ICBs, systems have found themselves with varying approaches to funding, activity and outcomes in the specifications for locally commissioned (enhanced) services across the geography.
    By undertaking an independent review underpinned by consultation with commissioning leads, clinicians and practice management, we will produce a set of recommendations to support the alignment of these schemes and identify opportunities for streamlined processes releasing vital capacity back to the system.
  • Using geospatial intelligence to support practice relocation and patients impacted by practice closures
