Population health

A focus on population health is now an established priority for the NHS with partners across Integrated Care Systems, particularly alongside Local Authorities and the voluntary, community, faith, and social enterprise sector (VCFSE).  

The re-emergence of a population health focus, partly in response to the Covid pandemic and built into the core purpose of ICSs formally created in 2022, is a welcome step - one that recognises the vital place for upstream action on health improvement, prevention, health protection, and reducing health and healthcare inequalities. 

We see population health as a wide and encompassing concept. At its core is an overriding focus on taking steps to improve the overall health of the population and reduce health inequalities across populations. It embraces key disciplines and activities of prevention (primary and secondary), public health, health promotion, health protection, acting on social determinants, and optimising access, experience, and outcomes for all through healthcare services.  

Specific approaches including population health management, proactive / personalised care, and co-production with people and communities are all important components to a population health approach.
