Our Primary Care team provides innovative and proactive consultancy services working in partnership with you.

Our approach builds on our experience in primary care development with the ability to draw in expertise as required from specialists in areas such as care analytics, organisational and leadership development, communications and engagement, evaluation and change management.

Partnership and PCN development

Delivery of facilitated sessions, co-designed with the practice or PCN to address challenges in leadership teams, partnerships and supporting PCN development.

Workshops

The development of PCNs provides the opportunity for the development of primary care and place-based services and will enable a greater connection with community, charity and voluntary sector services across an integrated neighbourhood.

PCNs need to consider new approaches to their workforce to enable more joined-up delivery to patients. Integrating these new roles into the primary care system effectively will take time to avoid duplication, achieve role optimisation and work effectively as a multi-disciplinary team.

NHS SCW workshops provide a fantastic infrastructure upon which peers can come together, build up a network of colleagues beyond the parameters of their PCN and share their experience and learning with others facing similar scenarios, and benefit from hearing and learning from others.

Shared problems and understanding are impactful ways of helping staff feel supported. This applies to the clinical directors and leadership team, and to all staff in the clinical and general running of practices.

Improvement and change management

ELITE programme

Transformational support to data-led improvement through our ELITE programme

  • Providing PCNs with a tool that allows them to easily access their operational data in real-time (Apex) and help them to understand and validate this data.
  • Taking PCNs through improvement cycles with a real project to unlock real benefits.
  • Build capability, understanding and data standards at PCN level to enable practices to begin to harness the benefits of working together.

Addressing the challenges

Addressing the challenges impacting on the experience of people accessing General Practice.
  • Development and delivery of PCN Capacity and Access Improvement Plans
  • Provision of Digital and Transformation Lead service with the ability to draw on a blend of expertise mirroring practice and PCN priorities to ensure delivery of plans
  • Mapping practice and PCN processes to inform improvement including ensuring the capabilities of team members funded through ARRS are maximised
  • Applying a user-centred design approach to website improvement, ensuring the experience of those accessing services is enhanced
  • Developing bespoke communications approaches to support patient engagement

Process mapping for efficiency

Mapping practice or PCN processes to inform improvement and driving efficiencies including ensuring the capabilities and impact of team members funded through ARRS are maximised.

The combination of the pandemic and the onboarding of new roles in General Practice at scale and pace at a time when capacity is stretched to breaking point has, in some cases, meant that changes have been implemented when teams are incredibly busy and has subsequently left them feeling less organised with less well-defined processes​.

The result of this can be that the full impact of some of these roles isn’t being felt across the PCN. Examples of a few of the processes we have previously mapped with practices include:

  • Actioning tasks
  • Repeat prescribing
  • Medical document management
  • Test result processes
Adapting our approach to reflect the number of people engaged in each process, we will tailor the mapping to meet the needs of the individuals, the practice and the capacity to engage using a combination of drop-in or 121 sessions.

The output of these reviews would be a list of co-developed recommendations.

Primary care workforce

As referred to in the Fuller Report, the SCW team have developed a robust approach to the use of data and insight to inform and support practices and PCNs to identify opportunities and future workforce needs.


Digital and Transformational Lead support

Following the addition of the role of a Digital and Transformation Lead (D&TL) to the list of roles reimbursable to PCNs through the ARR scheme.  SCW is providing support to PCNs and their practices to design and implement ongoing improvement to services,  including the co-design, development and delivery of capacity and access improvement plans as part of the 23/24 Capacity and Access Programme.

  • Design and implement focused recruitment campaigns
  • HR support with the novation of contracts and provision of TUPE advice to support the development of the workforce of the future
  • Using data and insight to develop workforce plans
