
Implementing a recovery action plan for Kent and Medway ICB All Age Continuing Care achieved a significant reduction in risk and a greatly improved service provision to patients
Objective
Kent and Medway ICB’s All Age Continuing Care (AACC) service was transferred to SCW in April 2022. A review of the existing service exposed significant operational failings which needed addressing.

What we did
We worked with the Kent and Medway ICB to understand the service and develop an agreed service Recovery Action Plan. Its implementation has achieved a significant reduction in risk and a greatly improved service provision to patients. Having successfully delivered a high-quality service, we are now implementing a sustainability plan to continue to embed continuous improvement.

Outcome
Work began in October 2022 in 4 areas with the following achievements delivered:
- Backlog of 949 patient reviews now cleared with no outstanding reviews remaining.
- 41 patients with outstanding care placements were triaged by clinical risk, reviewed and care arrangements put in place. This is now reduced to 1 complex patient where review of care provision is ongoing with the ICB.
- Approximately 900 outstanding ICB provider contracts reviewed and prepared, and on track to be issued by 1 April 2023.
- Backlog of approximately 5,000 unpaid invoices reduced to 579. All remaining invoices over 30 days are now dependent on external actions to resolve.
