A joined-up system approach to partnerships in Portsmouth

The Portsmouth Hospitals Trust was facing issues with contracting such as budget overspends and increasing demands. To overcome such challenges, we developed an Aligned Incentive Contract, encouraging collaboration and understanding across multiple organisations.

Objective

Working within a system with limited flexibility to address issues such as overspend and for contingency planning, the Portsmouth Hospitals Trust wanted a new approach that encouraged open, honest dialogue and encouraged organisations to work together and share risks.

What we did

Working with the Portsmouth Integrated Care Partnership, we developed an Aligned Incentive Contract, with the core tenet ‘the failure of one is the failure of all’, encouraging risks to be shared and managed together.

To support organisations to identify opportunities for collaborative working across the system, we developed a dashboard that presents data on referrals, outpatient activity, inpatient activity, waiting lists, and diagnostics from national, regional and local sources are included.

An accompanying briefing pack provides a narrative to add depth to the data which can be used to identify opportunities where collaborative working can support the system to develop solutions.

Fully tested model to forecast bed usage, discharges, and community demand.

Underpinning methodology enabled the model to be easily updated.

Outcome

  • A culture of discussion is now present within the joined-up and collaborative system, promoting greater transparency and enabling open and honest conversations to take place.

  • The availability of the dashboard has shifted the focus to investigating issues and developing solutions that work for the system and patients.

  • Success of this project has led to discussions around undertaking a similar project in Hampshire and the Isle of Wight.
